    Metformin versus glucophage

I'm seeing quite a few posts on BBSes from people who are having problems with metformin because of side effects that could be eliminated if they were taking the extended release form of this drug. For some reason, many family doctors don't seem to be aware that there is a ER version of this drug that has such benefits. This is probably because metformin is a cheap generic and isn't promoted by herds of beautiful ex-cheerleaders turned drug company salespushers who "educate" doctors about far more expensive--and less effective--newer drugs. Here are the facts: Metformin (also sold under the brand name Glucophage) comes in a regular version which is taken at meal time, three times a day, and an extended release form (marketed as ER or XR) which is taken once a day. Almost always, when people report diarrhea or intense heartburn with metformin, they are taking regular version. It was very disturbing because the pain was localized over my heart and felt just like the description of a heart attack you read in articles. My doctor assured me it was coming from the metformin, but that didn't make it any easier to live with because I kept wondering how, if I were having a real heart attack, I'd know it wasn't a pain from the drug? The ER version releases the drug more slowly and this usually eliminates the gastrointestinal problems. The UK Prospective Diabetes Study, a large clinical trial performed in 1980-90s, provided evidence that metformin reduced the rate of adverse cardiovascular outcomes in overweight patients with type 2 diabetes relative to other antihyperglycemic agents. Treatment guidelines for major professional associations including the European Association for the Study of Diabetes, the European Society for Cardiology and the American Diabetes Association, now describe evidence for the cardiovascular benefits of metformin as equivocal. In 2017, the American College of Physicians's guidelines were updated to recognize metformin as the first-line treatment for type-2 diabetes. For example, a 2014 review found tentative evidence that people treated with sulfonylureas had a higher risk of severe low blood sugar events (RR 5.64), though their risk of non-fatal cardiovascular events was lower than the risk of those treated with metformin (RR 0.67). There was not enough data available at that time to determine the relative risk of death or of death from heart disease. study known as the Diabetes Prevention Program, participants were divided into groups and given either placebo, metformin, or lifestyle intervention and followed for an average of three years. Metformin treatment of people at a prediabetes stage of risk for type 2 diabetes may decrease their chances of developing the disease, although intensive physical exercise and dieting work significantly better for this purpose. The intensive program of lifestyle modifications included a 16-lesson training on dieting and exercise followed by monthly individualized sessions with the goals of decreasing weight by 7% and engaging in physical activity for at least 150 minutes per week. The incidence of diabetes was 58% lower in the lifestyle group and 31% lower in individuals given metformin. Among younger people with a higher body mass index, lifestyle modification was no more effective than metformin, and for older individuals with a lower body mass index, metformin was no better than placebo in preventing diabetes.

Propranolol is a beta-blocker sold under the brand names Bedranol and Inderal. While it is often prescribed for conditions such as high-blood pressure, angina and irregular heartbeats, this scientific review will analyze studies on its effects in treating anxiety, the side effects and reported dosages. The beta-blocker works by blocking the action of organic chemicals, such as epinephrine. Therefore, propranolol has been explored as an off-label treatment for epinephrine-induced anxiety attacks or panic attacks. A study published in The Journal of Clinical Psychiatry, 212 patients were given propranolol, a benzodiazepine or a placebo. The study found that patients in the trial reported an improvement in anxiety based on the clinical scales when taking propranolol and the benzodiazepine. In the second week, propranolol was better than placebo based on two clinical scales used to measure anxiety. 6. Background: Use of penicillin class (such as ampicillin/sulbactam) is included in a treatment guideline of community-acquired pneumonia (CAP). The hospitals participated in this study were 4 academic hospitals in Korea. However, the use of the wide spectrum antibiotics such as 3generation cephalosporin is common in the clinical practice. Results: We enrolled 225 patients (ampicillin/sulbactam: 72, amoxicillin/clavulanate: 3, 3 generation cephalosporin were 1.17±1.05, 1.07±0.95 (p=0.487), respectively, and Pneumonia Severity Index (PSI) were 77.5±28.2, 74.7±29.3 (p=0.50), respectively. This practice might aggravate the problems of antimicrobial resistance and high medical expenses. We did random matching at a ratio of 1:2 (age; ±5 years, sex; same) for ampicillin/sulbactam or amoxicillin/clavulanate: 3generation cephalosporin as empiric antibiotics for CAP. However, there was statistically significant difference between both groups in the combination of macrolide or quinolone; penicillin group 68.0% (51/75) vs. 3generation cephalosporin was 2.7% (2/75), 1.3% (2/150) and was not statistically significant. Additionally we conducted multivariate logistic regression analysis with variables (empiric antibiotics, age, Charlson comorbidity score, presence of combination, PSI score, etc.).
